Line 1
Enter your complete legal name as used on your personal tax return.
Line 2
Enter your business name or the name of your disregarded entity if different from the legal name used on your personal tax return.
Line 3
Check the appropriate box for the U.S. federal tax classification of the person whose name is entered on line 1. Check only one box on line 3.
Line 4
If you are exempt from backup withholding and/or FATCA reporting, enter any code(s) that apply to you in the appropriate space.
Line 5
Enter your street address or the the address of the business entity listed on Line 2.
Line 6
Enter your city, state, and zip code or the information for the business entity listed on Line 2.
Part I
Enter your social security number or Employer Identification Number.
Part II
Thoroughly read the Certification to make sure you are truthfully completing the form.
Sign and date the form.
